Tomasoviæ-LonŁariæ3 1Department of Clinical Cytology and Cytometry, 2Department of Plastic, Reconstructive and Cosmetic Surgery, 3Department of Clinical and Experimental Pathology, 4Department of Nuclear Medicine, Dubrava University Hospital, Zagreb, Croatia AIMS: To evaluate the utility and accuracy of intraop- erative imprint cytology (IIC) of sentinel lymph nodes (SLNs) in patients with metastatic melanoma, and to es- tablish this diagnostic procedure in Croatia. METHODS: Seventy-two sentinel lymph nodes from patients with metastatic melanoma were identified by tech- netium-labeled sulfur colloid and blue dye. SLNs were evaluated intraoperatively by slicing into 3-mm sections. Imprints were made of each cut surface, stained with Hemacolor quick method and examined by a cytologist. Permanent cytologic imprints were stained with Pappen- heim and immunocytochemical staining for S-100 and HMB-45 proteins. Intraoperative imprint cytology results were compared with final histologic ones. RESULTS: A total of 72 SLNs were excised from 47 patients with malignant melanoma (1.6 SLNs per patient). Metastases were identified in 13 (27.5%) patients. The sensitivity of IIC for SNL was 75% (12/16) per sentinel node and 69% (9/13) per patient. All 4 false-negative SLNs by IIC were detected in patients with isolated cells of melanoma in permanent histopathologic sections. No false- positive results were identified (100% specificity). CONCLUSIONS: IIC of SNLs of metastatic melano- ma is a reliable and accurate intraoperative technique. It could be used as an alternative to frozen section, especial- ly be
