•Examined behavior problems among cocaine exposed children•Cocaine exposure predicted low baseline RSA and low RSA suppression at 13months.•Low baseline RSA at 13months predicted fewer behavior problems at 36months.•Maternal negativity moderated the relation between baseline RSA and behavior problems.
